What Elder Care Support Includes
Care Plan Support & Family Meetings
Our team can attend care planning meetings with families, assisted living facilities, skilled nursing facilities, and healthcare providers to help families understand recommendations and advocate for the needs of their loved one.
Care Monitoring & Family Updates
We help families stay informed by monitoring care progress and identifying changes in functioning, well-being, and overall engagement. Our team may also observe the physical environment, conduct inventories, assess facility programming, encourage participation in social activities, and advocate for any qualifying services such as PT/OT/ST. Following each observation, families receive a detailed email summary and a follow-up phone call to review findings and discuss updates that support informed decision-making.
Elder Care Navigation & Placement Guidance
We assist families in understanding available care options, including home care, assisted living, memory care, and skilled nursing services, helping them make informed decisions that best meet their loved one's needs. Our packages include care coordination and referrals, assisting in transition, and follow up to address any difficulties with adjusting.
Family Advocacy & Coordination
Our team helps bridge communication between families and providers to ensure that the goals, preferences, and safety of the older adult remain central to care decisions. This may include advocating for PT/OT and other DME in the home.
Short-Term Navigation Support
This service is designed for individuals and families of older adults living in the community or independent living settings who are noticing changes in cognition, emotional well-being, or physical functioning and want guidance on remaining in their current environment or exploring alternative living options.
Our licensed professionals conduct comprehensive assessments to help identify the safest and most supportive care arrangements, while also recommending services that can enhance quality of life. Each navigation package includes a dedicated transition specialist or social worker and a defined number of support hours to assist families during the transition process.
This structured approach allows CAPS-C to provide focused guidance while reducing fragmentation through coordination and communication across providers and service systems. Support may include:

CAPS-C’s navigation model is designed to be flexible, supportive, and responsive while keeping the older adult’s dignity, safety, and long-term stability at the center of every recommendation.

Guided Support
Best for: Families needing help exploring options
More hands-on support with evaluating and comparing care solutions.
Includes everything in Essential, plus:
- Assistance comparing care options (home care, assisted living, etc.)
- Coordination with 1–2 providers or facilities
- Up to 3 support hours for navigation tasks
- Ongoing email support during service period
Transition Support
Best for: Families actively making a care transition
Full support during critical changes like hospital discharge or relocation.
Includes everything in Guided, plus:
- Transition planning (hospital, rehab, or facility discharge)
- Coordination across multiple providers/services
- Up to 6–8 support hours for hands-on assistance
- Family guidance and decision support sessions
- Follow-up after transition to ensure stability
Comprehensive Care Navigation
Best for: Families seeking ongoing, high-involvment, support and advocacy
A concierge-level service that provides continuous guidance, oversight, and peace of mind.
Includes everything in Transition Support, plus:
- Dedicated social worker/transition specialist
- Attendance at care plan meetings and provider coordination
- Ongoing care monitoring and observation
- Regular updates and detailed progress summaries
- Priority access and extended support hours
- Family advocacy and communication support
- Enhanced support services including in-person or virtual check-ins and ongoing coordination through our Angel Partners model
