Our story
CAPS-C was created in response to a reality I witnessed repeatedly throughout my career in social services: individuals and families often face significant barriers when trying to access psychiatric and psychosocial services in a timely, coordinated way. Too often, care is fragmented across multiple providers, locations, and systems which creates delays, confusion, and added stress during moments when support is most needed.
With a master’s degree in social work and many years of hands-on experience in mental health practice, hospital case management, long-term care and home health, I founded CAPS-C to offer a more integrated solution. The vision was simple but essential, a need to reduce the burden of navigating multiple services by providing an all-in-one psychosocial service center designed around accessibility, coordination, and continuity of care.
CAPS-C is equipped to support individuals across the lifespan. Whether you are seeking guidance for a young child experiencing behavioral challenges, an adolescent having changes in mood or suicidal ideations, a young adult transitioning into independence, adults navigating mid-life changes, or exploring options as an older adult, our services are tailored to meet you where you are in life. We recognize that no two individuals have the same needs, circumstances, or paths to wellness.
It is fundamental to our work to acknowledge that limited resources, environmental disparities, and systemic barriers can significantly influence how individuals engage in treatment. Through comprehensive biopsychosocial assessments, our licensed professionals work collaboratively with clients to identify areas of need, develop individualized treatment plans, and navigate community resources that support overall goals and long-term stability.
A core pillar of CAPS-C is timeliness. When support is needed, waiting weeks for an appointment can be detrimental. CAPS-C is designed to respond quickly, sometimes in the same day you can be seen by a mental health provider, including weekends and after regular office hours. This ensures that care is available when it matters most.
Equally important is trust building. Our services are not cookie-cutter, nor are they based on a one-size-fits-all model. At CAPS-C, every individual is heard, respected, and supported. Our History
From its founding, we have prioritized collaboration with community partners to ensure individuals receive comprehensive, well-coordinated care. We recognize that meaningful progress often requires support across multiple systems including healthcare providers, families, community organizations, and workforce partners.
Our team works closely with referral sources, medical and behavioral health professionals, social service agencies, and community programs to coordinate services that support each client’s unique goals. Through these partnerships, we help ensure continuity of care, reduce service gaps, and create a unified plan that supports long-term stability and well-being.
By maintaining active communication with partner organizations, we are able to align treatment recommendations, psychosocial supports, and practical resources such as employment pathways and community services. This collaborative approach allows us to respond more effectively to complex needs while keeping the client’s voice and goals at the center of the process.
