Community and Service Partnerships
CAPS-C believes effective care extends beyond any single organization. Many individuals require access to resources and services that fall outside the scope of what one provider can offer. Community partnerships allow care to remain connected, responsive, and practical.
The purpose of CAPS-C’s partnership network is to collaborate with organizations that offer services or resources complementary to our psychosocial and assessment-based supports. Through these partnerships, individuals can be referred to appropriate services without unnecessary delays or fragmented handoffs.

How Referrals Work
Community partners may serve as a referral resource for individuals receiving services through CAPS-C, and CAPS-C may refer individuals to partner organizations when additional or specialized services are needed.
